As part of our Strategic Plan, Carpenter Hospice identified Community Engagement and Programs and Service Expansion as key strategic directions. With the goal of supporting these key strategic directions, Carpenter Hospice has been working towards developing services and programs to enhance palliative care in the Burlington community. Current research speaks to the increase in demand of services caused by the increased aging population. Research also states that when polled, 70%-80% of Canadians wish to die at the home but the reality is 67% die in hospital or long term care. With this disconnect, Carpenter recognizes the gap in resources often preventing people to stay at home. Carpenter Hospice is working with community agencies as well as the formal medical system to develop programs and services to address these gaps. To summarize, the mandate of the outreach program is to better support those living with a life limiting disease in the setting of their choice(i.e.home, hospice, etc.).
