Service Coordination is the central hub for a client's involvement in Brain Care Centre programming. The Service Coordinator (SC) makes referrals to external programs or to internal programs. The SC then follows up on how the clients are doing. A SC also works to advocate for clients, helps them to communicate with other professionals, links them with recreational activities in the community, and assists with necessary paperwork. Support Services - this service includes one to one sessions and groups designed to assist individuals, couples and families with discovering and utilizing resources and strategies for maximizing wellness after brain injury. This process may include identifying personal strenghts and needs of support, goal setting, information about brain injury, introduction to coping strategies and educational resources, and/or referral to community resources. Community Rehabilitation - therapists and community support staff meet individually with participants to set goals and work on skill development - including assistive device training, independent living skills, cognitive ability, general ability, memory, vocational interest, social and communication skills and domestic activities.
