9.0 Continuing Connections: enhances quality of life for younger residents of continuing care facilities with mental disorders & complex medical needs by promoting participation in social, recreational & leisure activities 10.0 Community Helpers: mechanisms to identify, support & train natural helpers in the community. These helpers are invited to the programs training modules and provided opportunities to learn more about mental illness. 11.0 Seniors Community Outreach (flood funding) links with existng programs and services with the goal of re-creating normalcy using a variety of supports and activities to reduce social isolation and increase education and independent living skills following a significant event (i.e. flood) 12.0 Grooup Home Management: Hunter House (total 3 group homes). CMHA provides operational management for group homes (i.e. lease, expenses) but no service delivery to residents.
