The Mental Health Transition Home provides both stabilization and skill development to adults with diagnosed mental illness in a home like environment through treatment, counselling and medication adjustment. In partnership with Yukon Mental Health Services, Health and Social Services, Challenge DRGprovides continuous operation and service 365 days per year of the Mental Health Transition Home. Challenge DRG works with the Department of Health and Social Services on the therapeutic component and general treatment of all residents as directed by the mental health nurse who will develop and maintain individual client case management plans that include clinical treatment plan, treatment of the mental disorder and development of mental wellness, pro social leisure activities, safety plan, life skills plan and transition to elf-sufficiency plan.
