Community Care Durham (CCD), is a multi-service organization that provides Home Support, Respite (Caregiver Relief), Assisted Living, Supportive Housing, Exercise and Falls Prevention classes, COPE Mental Health services for adults and their caregivers who have needs related to aging, physical and/or mental health. Home Management Services (Enriched Services) is a collaborative effort between Community Care Durham and Lakeridge Health to support the needs of high risk senior patients/clients who can be safely discharged home with a sustainable level of in-home community support while awaiting short listed long term care placement. Our services are provided free of charge or for a minimal fee and are administered through various service locations across Durham Region. CCD is committed to "Supporting People, Strengthening Community". Almost 144,000 volunteer hours was provided during the year.
